Lord of Hatred picks up after Vessel of Hatred, so it's not the sort of expansion you'll want to start cold. The story assumes you know what happened with Neyrelle, Mephisto, and the mess left behind. If you're sorting builds, stash space, or Diablo 4 Items before diving in, that's sensible, because the campaign moves quickly once Lorath pulls you back into the hunt. In quest databases, it sits after the base campaign and Vessel of Hatred, effectively continuing the main thread as Act VIII.
The campaign is split into six chapters, starting with The False Prophet and ending with And the Chains Fall Away. Across those chapters, there are 58 named main quests. Chapter 1 is a short opener with seven quests, moving from Nahantu and Backwater to the voyage toward Skovos and the city of Temis. Chapter 2 is the real bulk of the campaign, with 26 quests. It jumps between Lilith's memories, Rathma's trail, Morlu attacks, Amazon conflicts, Drowned forces, and the long push toward another piece of Lilith's Blade.
You'll notice the campaign likes to pull you in and out of strange spaces. The Sightless Eye, the Mind's Eye, Lilith's blood petals, and Mephisto's memories show up again and again. Sometimes you're escorting Lorath. Sometimes you're clearing Hellgates, chasing corrupted followers of Akarat, or solving a dungeon puzzle. The Path of Riddles is worth mentioning because patch 3.0.2 corrected a bad clue reference: players should follow "Corruption," not old notes that mention "Destruction." If a guide says otherwise, it's probably working from outdated information.
Chapter 3 is unusually brief, with only Faith and Failings, but it matters because Mephisto's influence becomes much more direct. Chapter 4 then opens things up again. You travel through Celestia, Skartara, Mount Hefaetrus, and the First Vault. This is where the Horadric Cube enters the campaign after the Horadrim quest and the Horadric Guardian fight. Before that, you deal with sigils tied to Jered Cain, Iben Fahd, Zoltun Kulle, and Tal Rasha. Later quests bring the Death sequence, Lorath's fate, and corrupted Amazon forces before the campaign turns toward Mephisto himself.
Chapter 5 sends you through Akarat's Bulwark, the Temple of Union, and the Pools of Creation before the Lord of Hatred quest pits you against Mephisto with Lilith's Blade. Chapter 6 is just No Gods, No Masters, though sources disagree on whether its listed location is Light's Bastion or the Tree of Whispers. Patch 3.0.2 also fixed several progression blockers, including one tied to Town Portal use during Death. Beyond the story, Lord of Hatred adds systems such as Talismans, War Plans, Fishing, Party Finder entries for Echo of Mephisto, and Cube crafting, so keeping useful D4 items ready can make the run feel far less awkward as the expansion opens up.
